Continuity of Care

Building trusted relationships with your care team.

Seeing the same clinician makes a difference

At our practice we aim to help you see the clinician who knows you best whenever possible.

Continuity of care helps us to provide:

  • More personalised care
  • Better understanding of your health history
  • Stronger patient–clinician relationships
  • Safer clinical decisions

What this means when you book an appointment

  • We will try to book you with your usual GP or care team
  • Patients with long-term or complex conditions may benefit most from continuity
  • If you need urgent care, another clinician may help you sooner

Our commitment to you

We are working to strengthen continuity of care so every patient feels:

  • Known
  • Listened to
  • Supported

Strong relationships between patients and clinicians help us deliver safe, high-quality care.

If you would like to request your preferred GP, please let our reception team know when booking your appointment.

What is the Continuity of Care:

It is when the patient sees the same GP (or members of a clinical team) repeatedly over time. Good quality care improves health outcomes and continuity of care is a crucial component of quality of care. General Practice is uniquely placed to provide continuity of care.

Our Continuity Vision

We want our practice to be a place where patients are not just appointments, but people we know.

In our future practice:

  • Patients can see their usual GP or care team whenever possible.
  • Staff work together to protect and prioritise continuity.
  • Our systems make it easier to build lasting patient–clinician relationships.
  • Patients feel heard, understood, and confident in their care.
  • This approach will improve patient experience, support safer clinical decisions, and create a more rewarding and sustainable way of working for our team.

Our guiding values are:

  • Compassion
  • Teamwork
  • Respect
  • Accountability
  • Patient-centred care

Together, we will make continuity of care a core part of how we look after our community.
